The ComboPlus Basic plan provides for generic drugs and dispensing fees, and standard dental work with recall exams every 9 months. In addition, Core benefits include vision, health care, travel, accidental death and dismemberment, and survivor benefits. There are a number of add-on benefits so that you can customize your policy to meet your needs.

Drug Coverage Coverage includes generic drugs (name brands are allowed, but coverage will be paid for the generic equivalent) and reasonable and customary dispensing fees. The following table lists the coverage amounts:

Drug Exclusions
Exclusions include smoking cessation drugs, over-the-counter drugs, fertility drugs, birth control drugs, and drugs not requiring a prescription.
Dental Benefits
Coverage includes fillings, cleanings, scalings, examinations, polishing, and some extractions, with recall visits every 9 months. The following table lists the coverage amounts:

Seniors' Adjustment
If you are 65 or older, your ComboPlus Basic drug benefits are as follows: 100% on first $500 of eligible prescription costs. 90% on next $2,000, to a maximum of $1,800. Full coverage of reasonable and customary dispensing fees.
